2025-26 WIAA PHYSICAL CARDS
Before a student can participate in interscholastic athletics, the WIAA and Grafton High School requires written proof of current physical fitness as determined by a Licensed Physician or Advance Practice Nurse Practitioner. A student-athlete will need on file a WIAA Physical form. Physical cards that will are dated April 1 and after will be good for two school years.

ACTIVITIES CODE, WIAA CONCUSSION AND WIAA ATHLETIC ELIGIBILITY FORMS
Each of the above noted forms must be signed by both student-athlete and parent each year. These forms will be emailed in the summer before fall athletic seasons begin and will be filled out electronically.

ATHLETIC PARTICPATION FEE
There is a $85 fee for each sport participated in (maximum of $170 per individual, and $425 per family). These may be paid in the GHS Office or at Registration.
