Participation in athletics is a great way to get involved at John Long Middle School. To participate in interscholastic athletics, there are FOUR (4) items that are required on file in the JLMS Main Office. They are due prior to the first day of practice, and the majority of them are completed in Skyward under the Online Forms section. 

(1) PHYSICAL CARD
This is the original up to date Physical Card that is signed by your physician within the last year.
Physical Examinations taken April 1 and thereafter is valid for the following two school years; physical examinations taken before April 1 is valid only for the remainder of that school year and the following school year.

(2) EMERGENCY RELEASE FORM - this form is located in Skyward/Online Forms
This card includes information such as family doctor, emergency contact phone numbers, etc. which is used when the parent(s)/guardian(s) is not in attendance.

(3) CONCUSSION AGREEMENT - this form is located in Skyward/Online Forms

(4) ATHLETIC FEE
  $ 60 per sport
  $ 120 cap per student per school year. (3rd sport participating in is not charged)

Athletic fees can be paid via eFunds. If you don't see the fee in there, you'll have to add it to Skyward/Fee Management/Add a Fee.
Checks are also accepted in the school office.
