Transition (for ages 14-21 - preparing for life after high school)
- CCOT - County Communities on Transition - a regional group that comes together regularly to identify issues, barriers, supports, & solutions to assist students with disabilities make a successful transition based on postsecondary goals related to training or education, employment, and independent living.

- Portal Inc. - located in Grafton, offers many services to help individuals with disabilities to develop independent and interdependent life skills for quality living.
- Division of Vocational Rehabilitation (DVR) To apply for DVR services complete the online referral/application.
- Harrigan Development Services - social skills groups, vocational skills training, individual support services, disability consulting and case management, counseling for individuals and families, mentoring/daily living skills training.
- Ozaukee Youth Apprenticeship - Wisconsin's Youth Apprenticeships integrate school-based and work-based learning to provide students in their junior and senior years with academic and occupational skills leading to both a high school diploma and a Certificate of Occupational Proficiency in a specific industry.
Adult Services
- Opening Doors: A Guide to Adult Services
- Aging & Disability Resource Center (ADRC) - a division of the Wisconsin Department of Health Services where you can receive accurate, unbiased information on all aspects of life related to aging or living with a disability.
- Include, Respect, I Self-Direct (IRIS) - another division of the Wisconsin Department of Health Services that is a Medicaid Home and Community-Based Services (HCBS) waiver for self-directed long-term supports.
- Balance Inc. - primary objective is to provide support to people with disabilities in their homes, assist them in discovering and connecting with their community, and help them develop relationships that will enhance their lives.
